UPS returned 403 - causes free shipping

In the last few days i have received orders that somehow got through with free shipping. I tried it myself and found that if i go to Checkout and put in my shipping address, the shipping dropdown gets filled properly. But if I refresh the page then sometimes it is filled with this:
UPS - Price: $0.00
USPS Media Mail - Price: $5.30

I found this in the error logs:

>> DESCRIPTION:
UPS - Error: The remote server returned an error: (403) Forbidden.



What would be causing this, what do I need to change?
